Transport Request
PCHS transports dog and puppies to the Des Moines, Iowa and Minneapolis, Minnesota areas on a scheduled basis. Please complete the form below to request to get a pet on the transport. We prefer to receive the requests 4 days in advance, but will consider all requests until the transport van is full. We will reply within 1-2 days to let you know if we have room for your pet.

Transport Day Schedule:
7 am - Pick up at Polk County Humane Society in Bolivar, MO
9 am - Pick up at Winding River Pet Village in KC, MO
1 pm - Delivery at PetSmart in Clive, IA
6 pm - Delivery at PetSmart in Richfield, MN
Transport Requirements
VACCINATIONS:

Adult dogs must be up to date on Parvo/Distemper, Bordatella and Rabies vaccinations.

Puppies must have had at least 2 sets of Parvo/Distemper vaccinations and a Bordatella vaccination and must be at least 8 weeks old.
HEALTH CERTIFICATE:

Each pet must have a health certificate issued by a veterinarian.
IDENTIFICATION:

Each pet should have a collar with their name and the receiving party's contact information written on it.
PAPERWORK:

Each pet should have two copies of their paperwork in two separate envelopes. One copy is for the receiving party and one copy is for the transport. Both envelopes should be labeled with the pet's name, the receiving party's name and contact information.

The paperwork should include the medical record (vaccinations, surgeries, preventatives, etc.), the pet's health certificate and the pet's microchip information, if applicable.
DONATIONS:

We request a $75 donation for each pet to be transported, or $125 for a litter that can travel in one crate. This helps with the costs of the transport. Thank you!
